  1. Clear program model: What are the program’s eligibility criteria, goals and objectives, resources, outputs, and expected outcomes?  
  2. Stable and fully implemented program: How long has the program operated, and how stable has it been?  
  3. Sufficient numbers of participants: How many participants were enrolled (or do you expect to be enrolled) in the program? What are some of the reasons the program does not have (or does not expect to have) enough participants during the specified program time frame?   
  4. Data capacity: What data are currently collected? Who collects the data? Where are the data stored?  
  5. Leadership support for the evaluation: Do program and organizational directors understand and support the evaluation? Are they aware of the resources and information needed to evaluate the program?
